We are currently looking for 2 experienced registered nurses to join our community nursing services. These posts are fixed term until March 2022 to cover maternity leave within the team. They could also be offered as secondment opportunities.
This is an exciting time within Caerphilly as we look to develop a Graduated Care model within the Borough utilising both home based and community based services. This is alongside other developments within the team.
The successful applicants will be initially based within the Community Resource Team (CRT), which is a large multi-disciplinary team providing a 'hospital at home' service, aiming to prevent unnecessary admissions to hospital as well as facilitate early discharge. The Team has several other services including Reablement and an Emergency Care at Home services.
The development of the Graduated Care Model gives the Band 6 Nurse Practitioners the opportunity to work within the Redwood Suite which is a 12 bedded unit in the North Resource Centre in Rhymney. This opportunity will allow further development of clinical skills and managerial experience whilst providing a seamless service to meet our population’s health and social needs. This will be done on a rotational basis with all the Band 6 Nurses in the Team.
These are exciting and dynamic roles offering an opportunity for the right candidate to develop autonomous practice, as well as development and training opportunities.
You will need to be a qualified nurse who is committed to providing excellent care and who is dynamic and motivated in driving future practice within the community setting.
In return you will be supported by a very experienced team of nurses, including Advanced Nurse Practitioners, as well as Doctors and Consultants. You will also be part of the wider team with the ability to draw on the other professionals within the service.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
