Blaenau Gwent Community Resource Team is a fully integrated team providing services on behalf of Blaenau Gwent County Borough Council and Aneurin Bevan Health Board.  We are looking for a skilled and experienced Occupational Therapist to work with our patients/citizens in their own homes and communities as well as in-reaching into the Community Hospital.

Our vision is of a highly skilled, experienced, dynamic and flexible workforce that has the ability and confidence to work across our community to meet a person's ongoing occupational therapy needs through seamless transfer of care.

You will be expected to demonstrate your underpinning knowledge in occupational therapy philosophy, process and practice in the following areas:

- community rehabilitation

- multi-disciplinary and inter-disciplinary working

- equipment and adaptations to people's homes

- admission avoidance

- rapid hospital discharge planning

- palliative care

- discharge to assess

- in-reach to hospital

- working collaboratively with third sector and voluntary services
