An exciting opportunity has risen for an experienced Band 7 Nurse, to lead on a new project.
The opportunity could be as a secondment or fixed term until 31st March 2020.
The primary purpose of the team is to provide support to GP practices within North Torfaen with home visits for patients who are resident in nursing and residential care homes.
A large cohort of Frail and Elderly patients can remain at their usual place of residence if they have access to additional support in the community. The team will help to ensure that the patient remains well in their usual place of residence as well as ensuring that ACP (anticipatory care plan), planning is in place or being undertaken with the patient and relatives (where appropriate)
The Transformational Team will provide a Comprehensive Geriatric Assessment (CGA) for every patient, as well as starting or continuing ACP planning to ensure that the patient’s wishes are captured.
The team will ensure that the patient will receive the right care, at the right time in the right setting; ensuring that patients’ receive care in a limited amount of ward/care setting transfers as possible.
If the team note an acute problem with the patient, they will be managed within the CRT usual processes, i.e Rapid Medical or Rapid Nursing to support the patient in their own home as much as is clinically appropriate.
Patients reviewed under this model will have timely access to CRT services and telephone advice from Consultant Geriatrician (if appropriate).
The Team will not complete elements of care in direct relation to the DES for care homes, the GP will still need to review new patients under the DES contractual arrangements.
