We are seeking an experienced and motivated Band 6 Deputy Case Manager to provide clinical expertise and leadership within the Continuing Healthcare Complex Care at Home Team. This is a challenging but rewarding role that requires you to demonstrate excellent communication, leadership and motivational skills.

Your role will be to support the Case Manager/s to provide the highest level of quality care to patients with complex needs within their own homes over a 24 hour period. You must be driven to achieve and sustain high clinical standards, be a credible role model and have the ability to influence others.

You will need to be well organised, proactive and confident in your own abilities and bring with you a portfolio of relevant extended skills. You need to be proactive and innovative with the ability to meet changing needs and the ability to work on your initiative and manage your own workload effectively.

The role is both challenging and rewarding in return we can offer you a robust induction programme and opportunities to learn new skills.

The ability to speak Welsh is desirable for this post.
