We are currently looking for highly motivated, dynamic Band 4 Assistant Practitioner to join our busy Community Resource Team in Newport.
The Community Resource Team’s (CRT), function is to work with people to enable them to be supported in their own home wherever possible, this will include a range of interventions The Team is made up of a range of professionals from Health & Social Care.
You will assist the Rapid Medical/Nurses, Reablement and Falls Team in delivering prescribed clinical interventions and support using good problem solving, time management and assessment skills and supervisory skills in a variety of community settings.
We aim to deliver high quality health and social care services for adults in the Newport and the role is an integral part of the CRT.
