Specialty Doctor in Intermediate Care

Based Pan Gwent Frailty Programme, Main Base at Newport Community Resource Team, South East Wales

  

An exciting opportunity to be part of the well established Community Resource Team at Newport is now available. This is a permanent post in an intermediate care setting based on Holistic Frailty care model of the Gwent Frailty Programme. You will experience and deliver Comprehensive care for the frail elderly working at the interface between Hospital and Primary care. Newport CRT is a fully integrated team of Health & Social care professionals. Your role would be to provide Medical input for the team in delivering acute assessments and Hospital at home interventions in the community avoiding unnecessary Hospital admissions of Frail individuals.  You will also be part of the innovated development of futuristic models of community "Care Closer to Home".

 

Free parking is available across all Aneurin Bevan University Health Board Sites.

This position involves Regulated Activity with children and adults as defined by the Safeguarding Vulnerable Groups Act (amended by the Protection of Freedoms Act 2012) and the following checks will be undertaken following any conditional offer:

Applicants must hold or be eligible to hold full registration and a licence to practice with the GMC Candidates.
