An exciting opportunity has become available for a qualified mental heath nurse to join Newport East Community Mental Health Team based in Goldtops Newport as a Band 6 CPN. Newport CMHT are a dynamic, multi disciplinary team offering a wide range of nursing, vocational and therapeutic interventions to service users experiencing complex mental health problems.
The successful post holder will become a member of an integrated, multi disciplinary mental health team. They will be responsible for the care coordination of a caseload of service users who are experiencing severe mental health difficulties from assessment to discharge. Will take responsibility within their caseload for ensuring the assessment of health and social needs, formulation, planning, delivery and evaluation of care, support and treatment by the team, for both individuals and groups of service users including risk assessment and management, CTP, and care reviews.
They will work collaboratively across a wide ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area and will provide cover on a rota basis for Newport's Mental Health Duty Desk dealing with urgent queries, referrals and advice calls.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
