An exciting opportunity has arisen for a full time Registered band 5 within a supportive friendly district nursing team based in the vibrant city of Newport.
Central East District Nursing team Newport is part of a two year project to pilot a new model of care for district nursing services, a neighbourhood district nursing model. We are focusing on the Neighbourhood based system of district nursing care, with a focus on continuity of care delivered by a self managed team of nurses.
Central East District Nursing team is one of ABUHB pilot sites to test the model in a Welsh Urban setting.
As a team we have had a shift in services and resources to provide care closer to home with district nurses playing a crucial role working as partners to empower patients to supportively manage their own health and well being with focus on early intervention prevention and promoting independence. Delivering a safe, effective, alternative to hospital admission.
Central East is a fast paced and progressive team that challenges the traditional model of district nursing having been given the springboard opportunity of being part of the 'Neighbourhood Nursing' pilot. Allowing the team to bridge the gap between community and acute, bringing care closer to home in a population with more complex needs.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
