We are looking for a dynamic and self-motivated individual to join a busy Clinical Commissioning Team  (Mental Health & Learning Disabilities) as a Patient Flow Coordinator. The role will be pivotal in joining up all external partners from local authorities, Housing Associations and third sector providers to ensure that, when a person is medically fit for discharge, all welfare matters have been addressed to ensure a safe, rapid discharge. 

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
