An exciting opportunity has arisen for an experienced nurse to job share a community nursing post and work with North Monmouthshire Integrated Team based at Monnow Vale Health and Social Care Facility.
The aim of the service is to provide a proactive, dynamic, skilled, respiratory/oxygen service for patients within their own home or closer to home at the integrated hubs of Monnow Vale and Mardy Park. This will ensure that the Care Closer to Home Strategy, Clinical Futures and clinical guidelines outlined by the British Thoracic Society and Aneurin Bevan University Health Board are delivered in a coordinated way across Monmouthshire.
The successful candidate will be a nurse with the relevant experience, who has excellent respiratory skills and experience with chronic disease management particularly heart failure and diabetes.
The candidate must be able to demonstrate effective organisational skills, is an excellent communicator, team player and ideally has experience of community-based working and case management.
The successful candidate will job share with the existing post holder and be supported by Monmouthshire Integrated Team, Monmouthshire GPs and respiratory consultants based at Nevill Hall Hospital, Abergavenny.
