Based Chepstow Community Hospital

Chepstow Community Hospital is a Community Hospital setting, which is considered a real asset to the local community. The hospital provides care to the population of the surrounding Chepstow/Monmouth areas.

There is one 32 bedded unit consisting of predominately Care of the Elderly patients who are undergoing a period of rehabilitation and discharge planning.

A recent successful pilot held at Chepstow Community Hospital means any successful candidate will be an integral role in the management of direct GP admissions, avoiding unnecessary admissions to the local General Hospitals. The post is full time and and there is no current need to join the on-call rota

Educational opportunities are excellent with an active teaching programme in the local general hospitals and further community hospitals.

The post holder will experience a very broad spectrum of clinical experience and a collaborative interface with wider associated partners within medicine, Local Authority and Third sector organisations, aiming always to work together to provide an excellent patient experience.

This position involves Regulated Activity with adults as defined by the Safeguarding Vulnerable Groups Act (amended by the Protection of Freedoms Act 2012) and the following checks will be undertaken following any conditional offer:

  • Enhanced check with barred list information, including   
  • - an adults barred list check

Applicants must hold or be eligible to hold full registration and a licence to practice with the GMC.
