The Welsh Government Primary Care plan for Wales has recognised that “no GP should routinely be undertaking any activity which could, just as appropriately be undertaken by a pharmacist”.
The Neighbourhood Care Networks (NCNs) of Aneurin Bevan University Health Board have embraced this opportunity and are looking to recruit additional practice based pharmacists. You will work closely with the NCN Lead and be a core member of the practice team working closely with other health and social care professionals.
Based in Chepstow, the successful candidate will be responsible for optimising the care of patients medicines within a GP practices, supporting activities such as medicines reconciliation, drug monitoring and undertaking medications reviews.
The aim is to release GP capacity within the surgery and to extend the pharmacist role within a primary care setting.
This role may include the opportunity to train as an independent prescriber.
The ability to speak Welsh is desirable for this post; English and/or Welsh applicants are equally welcome to apply.
