THIS POST IS FIXED TERM FOR 2 YEARS DUE TO MEET THE NEEDS OF THE SERVICE
This post will be offered as a band 7 or band 8a depending on experience.
Are you looking for a new challenge? Within ABUHB, two Neighbourhood Care Networks (NCNs, or GP clusters) are looking to recruit practice based clinical pharmacists. The aim is to release GP capacity within the surgery and to extend the pharmacist role within a primary care setting. You will work at each practice within the NCN, working closely with the NCN Lead. You will be a core member of each GP practice team, and work closely with other health and social care professionals.
Posts are based in Caerphilly East (1 WTE) - Risca/Newbridge/Blackwood area
Individuals will be responsible for optimising the use of patients' medicines within GP practices, undertaking activities such as medicines reconciliation, face to face medication reviews, drug monitoring and running clinics.
You must a registered pharmacist with the General Pharmaceutical Council (GPhC); and you should be able to demonstrate excellent organisational and time management skills. The successful candidate will be able to prioritise workload, work independently, build key relationships and be confident interacting with a wide range of stakeholders such as GPs, practice nurses, secondary care clinicians and patients.

We are looking to recruit to a band 8a salary for those applicants who meet the essential qualifications and experience. However, we will consider recruiting applicants at a band 7 with less experience and working towards their diploma
Applicants are expected to be able to travel across ABUHB office bases and to GP practices. Expenses will be reimbursed according to the organisation’s policy
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
