“We are advertising for a part – time Band 5 nurse to work in the Discharge Liaison Team in Monmouthshire based in NHH. You will work alongside a Band 7 nurse and a support worker. Monmouthshire Discharge Liaison service is part of the Monmouthshire Integrated Services and operates a model based on planning discharge as soon as someone presents to the hospital this may be arranging their discharge straight home from A&E / EAU or co-ordinating the discharge of more complex patients – attending MDT, liaising with families, arranging transfer to a community bed, re-patriating a person from an out of area hospital.

The service is a pro-active, fast paced service that works very closely with MDT members of the integrated service in the community.

We are looking for someone who likes a challenge, can work in a pressured environment but always has the needs of the person at the heart of all they do.”

The ability to speak welsh is desirable for this post.
